Transition from the teenage years into adult life
Thinking about leaving school can be very exciting for teenagers. It’s a time of new opportunities, new challenges, a time to think about the future and the skills they have and need to develop.
It’s a time where services change and young people can start to take responsibility - where this is possible - around their health and wellbeing and the way they choose to live their lives.
Planning needs to start early - from about Year 9 of high school. To help with this, a Transiition Toolkit has been developed for young people with cerebral palsy in New South Wales.
The Toolkit is a series of 21 factsheets designed to cover some of the key considerations for young people as they move into adult life.
