TASC is a consultancy service and requires active input from the client’s local team. TASC can provide an assessment for the client, or a therapy consult for the local therapist.
The local therapists remain the client’s primary therapist throughout the assessment process. It is the responsibility of the local team to support the client during the assessment, the trial of equipment, support the client through the funding process as required and to implement the equipment once funding is received.
TASC seating requires the client’s primary therapist to make the referral.

Contacting TASC for a referral
Contact TASC by phone or email to request a referral.
A TASC therapist will contact the referrer via phone to take detailed referral information. This screening process can take 10-15 minutes (technology) and up to 30 minutes (seating).
Following the telephone screening, intake forms and a letter confirming the goals identified will be sent to the referrer. The client will be sent consent forms and a Spastic Centre Service Contract. The intake form is to be completed by the local therapist in consultation with the client and relevant team. This form requests additional and more specific information such as measurements of the existing seating and environmental considerations (seating), access methods to the computer and details of current communication system (technology). These forms are to be returned to the TASC team prior to an appointment.
